HOW TO GET YOUR POLAR HRM SERVICED
Please use this form when sending your Polar Heart Rate Monitor for service. Print this page; fill out the requested information and return with your complete unit. We always recommend that you use a traceable and insured shipping method, such as UPS or Insured US Mail. Polar is not responsible for items lost in transit.
NOTE: You can expedite your repair by pre-authorizing repair charges. The amount we recommend is $59 which will cover receiver battery and replacement transmitter (most common repairs). Please include a credit card number (Visa, MasterCard or American Express) with expiration date and authorize charges to $59. You will only be charged the cost of the actual repair. If the repair cost is more than $59, we will contact you by mail or e-mail.
All repairs come with a 90-day warranty!

Sending Instructions

Pack the product carefully to avoid damage. A box is recommended.

Include all the parts of the Polar Heart Rate Monitor. A full periodic check will be done on the whole product.

Include proof of purchase (a receipt or its photocopy) if the product is under warranty. THE WARRANTY PERIOD IS 2 YEARS FROM DATE OF PURCHASE on both the receiver and transmitter (for any units purchased after September 2002). Units purchased before September 2002 have a warranty of 1 year on the receiver and 2 years on the transmitter.
